CCR inspector arrived during naptime. Upon arrival a 13month was transitioned from one classroom to another. Upon entering the classroom the caregiver stated that she had 9 children in care ages 12-24 months of age, not including the 13 month old recently transitioned from the room. This was immediately corrected at inspection by placing the child in another classroom.
During the course of the investigation it was determined that the operation failed to provide parents a written or electronic infant daily report on numerous occasions.
Potty chairs in the potty training room were not sanitized after each use.
The portable sinks in two of the classrooms had clogged drains and the dirty water was backing up into the sinks that were use for handwashing.
Loose fill surfacing is not maintained at six inches in the use zones for equipment that bounces.
Two containers of disinfecting wipes were found laying on a high chair within the reach of children. An outlet cover was observed missing in one of the classrooms. One diaper changing pad in a classroom was not intact. This was immediately corrected at inspection by removing the disinfecting wipes and adding an outlet cover. A brand new diaper changing pad was brought into the classroom.
During the inspection the playground had a tripping hazard for a play deck area with boards sticking up presenting a tripping hazard. Also, the mulch was not covering the under surfacing presenting a tripping hazard to the children in care.
During the inspection a 4 month old child in care was being fed by a caregiver in a bouncer. This was immediately corrected during the inspection by holding the 4 month old child while feeding.
It was determined throughout the course of the HHS Investigation that a child in care was left alone on the enclosed playground unsupervised by the former caregiver for approximately 1 minute. The incoming caregiver was not given a report or a head count during the transition period.
During the unannounced follow-up inspection a 3 month old infant was observed sleeping in a swaddler. This was immediately corrected by removing the swaddler, placing the operation back into compliance.
A 4 year child was observed to be left alone in the front office, standing at the main entry door. It is unknown how long the child was left unattended.
It was determined during an unannounced Investigation Inspection by the DFPS CCI that a file for a former child in care could not be located.
It was determined throughout the course of the DFPS Investigation that the caregiver did not follow the medication label in terms of frequency with a child in care.
It was determined throughout the course of the DFPS Investigation that a caregiver failed to use good judgment by forcefully grabbing children in care.
During the unannounced inspection, a 7 month old infant in care was observed sleeping in a swing. The caregiver stated the infant had been asleep in the swing for a duration of 10 minutes. This was immediately corrected during the inspection by placing the infant in the labeled playyard.
The caregiver in the infant room stated there were 7 infants in care when there were actually 8 infants in care.
During the inspection 2 arch climbers were observed in the playground area.
During the unannounced DFPS Investigation Inspection, one personnel record did not have the required prohibited discipline available for immediate review and evaluation.
Based on the information obtained throughout the course of this investigation, it was determined that a 5 year old child was in the infant classroom.
