It was determined through the course of the investigation that a caregiver demonstrated a lack of good judgment by holding the glass classroom door closed for approximately 21 seconds, preventing a two-year-old child from entering the classroom while the child attempted to open the door.

It was determined through the course of the investigation that a caregiver acted in an impatient, irritated manner when changing a 9 months old diaper by swatting at the infant in the face with the infants pants, aggressively slamming down the wipes container on the changing table, and using quick, rough, non-nurturing movements with the infant while changing their diaper.

A caregiver was seen in video footage aggressively pushing a child away from the bathroom door in the classroom.
Video footage was observed showing a caregiver forcefully keeping an 18 month old child in time-out for a period of at least 20 min.
Video footage was observed showing a caregiver acting in an aggressive manner toward a child in care. The caregiver can be seen picking child up by one arm to place in timeout. grabbing arm to move away from a toy, and forcefully slamming the child to the floor to again place in time-out.

An infant room with 7 infants was observed to not have 2 children buckled in their seats. Children were sitting in the kidney bean feeding tables.
A fire extingusher on the Joy bus was observed to be out of date. The latest date on the tag indicated it was last inspected in 2012.
Three of the 2 year old classrooms were observed to have cleaning supplies and sharp scissors in cabinets located under the sinks that were not locked and in reach of children. A potty chair in another 2 year old room had dired urine in the potty. This was all corrected at inspection by moving all the items out of the reach of children. Placing locks on the cabinets and sanitizing the potty chair.
2 children with food allergies did not have a food allergy emergency plan signed by a physician.
A infant room with 8 infants was observed to not have any children buckled in their seats. All children were sitting in the kidney bean feeding tables. 2 seats were missing straps. This was corrected at inspection by replacing the missing straps and buckling in all the children at the tables.
