A 6 month old and a 3 month old were observed in a restritive device in the infant room. When asked the caregvier stated the infants had each been in the devices for about 20 minutes. This was corrected at inspection when the infants were removed from the devices brining the operation into compliance.

An 3 month old infant was observed asleep in a swing. The caregiver stated the infant had been asleep for 45 minutes. THis was corrected when the infant was removed from the swing bringing the operation into compliance.
Window cleaner was observed in the restroom in the drawer in access to children. The drawer did not have a lock on it. This was corrected when the window cleaner was removed from the drawer bringing the operation into compliance.
Portions of the chainlink fence are not anchored to the posts causing a hazard to children in care.
A 7 month was observed in their crib with a hand cloth sized security blanket. This was corrected at the inspection when the item was removed from the crib.
11 children ages 14 months to 21 months were observed to be unbuckled in the kidney bean toddler table during meal service. This was corrected at inspection when all the children were buckled in bringing the operation into compliance.
A caregiver did not have a valid CPR certification available for review during the time of the inspection.
Cleaning supplies were observed in multiple unlocked drawers in the restroom accessible to children in care. Furthermore, baby powder was observed under the changing table in the 2 year old room accessible to children in care. This was corrected at inspection when the cleaning supplies were removed from the drawers and the powder was placed out of reach of the children in care.
A caregiver in the Pre-k classroom was present at the operation before receiving notice from the CBCU regarding the their eligibility. This was corrected at the inspection when the caregiver left the operation and the operation submitted a background check on the caregiver.
It was determined during the course of the investigation that the operation did not provide the parent with an incident report for a situation that placed a child at risk.
It was determined that the operation failed to notify the parent immediately of physical abuse allegations against their child as required by Texas Family Code ?261.001.
It was determined through the course of the investigation that a child left their classroom without the knowledge of the caregiver. The child exited the building with an off-duty caregiver that was unaware the child had followed them out of the building. Therefore, there was a period of time where no one was responsible for the child in order to provide the level of supervision necessary to ensure each child?s safety and well-being.
It was determined throughout the course of the investigation that a caregiver hit a child on the hand in an effort to punish the child.
I was determined throughout the course of the investigation that the operation was not following background check requirements for 2 caregivers.
Multiple classrooms had cleaning supplies in reach of children on countertops and under cabinets. This was corrected at the inspection when the cleaning supplies were placed out of reach of the children in care.
Several staff records were missing required items such as affidavits, educational documentation and copies of photo ID's.
A caregiver was observed out of their classroom to open the main entrance door for a delivery driver upon my entry for the inspection. The caregiver then entered their classroom. The classroom had 10 children with ages 11 months to 17 months. At the time only one caregiver was present until the other caregiver entered back into the calssroom.
A 14 month old child was observed not buckled into the highchair during snack time. This was corrected at inspection when the child was buckled in.
A portion of the wall in the large playroom was missing paint/coating exposing the sheet rock and causing a hazard to children in care.
The operation had not documented a fire drill since 12/7/2022. This was corrected at inspection when the operation conducted a fire drill during the inspection.
Upon reviewing documentation of drills it was discovered that the fire drill conducted for July 2022 was not documented on the drill documentaiton form. This was corrected at inspection when the director documeted the fire drill on the form.
A caregiver in the infant room was not aware of how many infants were present. When asked the caregiver stated 8 children were present. Upon counting, there were 9 children present. A caregiver in the two year old room was not aware of the ages of children in their care
A classroom was observed to not wash hands after entering the room from participating in outdoor activities.
Diaper changing was observed in the toddler room: A caregiver was observed leaving a child unattended on the diaper changing table. The diaper changing table was not sanitized after use.
A 12 month old child in the infant room was observed to be unbuckled in a high chair. This was corrected at inspection when the infant was removed from the high chair. A 18 month old in the toddler room was observed to be unbuckled in the toddler kidney bean shaped table. This ws corrected when the child was removed from the toddler table.
A caregiver not current in pediatric first-aid with rescue breathing was observed unsupervised caring for a group of children in care. This was corrected at inspection when another qualified caregiver stepped in with the group of children. Bringing the operation into compliance.
As of the date of this inspection, the center is currently waiting on the city to conduct a sanitation inspection. This inspection should take place within the coming days.
