Upon review of files an employee was missing an affidavit for employment and three employees were missing proof of education qualifications.
It was determined while reviewing video footage that a caregiver can be seen exiting the classroom, leaving another caregiver alone to supervise 21 three and four year old children. This occurred twice on the same day. The first time for 21 seconds and the second time for 26 seconds.

A group of 33 children were observed to be playing on the playground. The youngest child was 13 months of age and the oldest child was 32 months of age. This was corrected at the inspection when the oldest group of children left the playground.
It was determined throughout the course of the DFPS investigation and after review of video footage that a caregiver lacked good judgment and self-control with a child in care when she used her arm to shove a child to the floor and then failed to comfort the crying child afterwards. The caregiver also used her hand to push a child away from her after talking to the child.
