It was determined through the course of the investigation that a caregiver pulled a 21 month old child by the arm in an effort to discipline the child.

A child with a doctor note that states the child has a dairy allergy does not have an allergy emergency plan on file.
A 10 month old infant not yet on table food did not have up to date feeding instructions. the form was last updated in March of 2023.
During outdoor time there was a 34 month old combined with a 12 month old on the playground. This was corrected at inspection when the 12 month old was removed from the group.
A caregiver in the infant room answered their personal cell phone during the walk through of the infant room.

A staff member that has access to the children was present at the operation before receiving notice from the CBCU regarding the their eligibility. This was corrected at inspection when the staff member left the premises.
2 caregivers were supervising 22 children with the specified age group of 23 months. This was corrected at inspection when another caregiver entered the room.
An infant was observed not buckled into the toddler table while eating snack. This was corrected at the inspection when the chld was buckled in by a cregiver.
It was determined through the course of the investigation that the operation failed to report suspected abuse of a child within a 48 hour time period.
The surfacing under the outdoor play structure has not been maintained at the required depth.
A 16 month old infant was napping in a reduced ratio classroom, along with children that are 18 months older than the infant. This was corrected at the inspection when the child was moved to the infant room.
A portion of the chainlink fence on the playground has separated from the frame causing a possible entrapment hazard.
Upon review of children's files, it was observed that 2 children with food allergies did not have a signed allergy plan by the parent or the physician. Furthermore, allergy lists were not posted in all areas that the children spend time.
