A child was left on the playground for a brief period of time and the parent was not notified until two and half hours later.
A two year old child was left unattended on the playground for approximately a minute and a 1/2. He was unharmed and brought back inside by a caregiver to rejoin his group.
A child who was upset and crying in pain reached out to a caregiver for help and the caregiver pushed him away and made no effort to see why he was upset.
Two caregivers sitting in the classroom could see that a toddler was visibly upset and crying. The child cried and appeared to be in pain. The caregivers allowed him to cry out for a minute and a half before picking him up to assess what could be wrong.
A two year old child walked into a restroom and was unattended for approximately four minutes when the group was transitioning from the gym to the classroom.
Based on information obtained from a DFPS investigation, a caregiver was not properly supervising a child in care that resulted in a misdiagnosed injury and the child being sent home without the appropriate treatment.
