It was found during the investigation that foster parent hit the child in care on the leg.

It was determined that two children in care were sharing a bed. Bedroom used by children has enough space to allow two small beds or a bunk bed.

During inspection of the backyard, a chair was observed to be by the entrance of the trampoline that is used at times for the children to enter and exit the trampoline. This was moved and corrected at inspection.

During walk through it was observed detergent and gasoline on garage floor. There was also bug repellent on a table in the backyard's porch as well as a propane tank next to the barbeque grill pit.

2 out of 8 home records did not have supervisory completed with both caregivers every six months. There was no documentation provided in the superisory visit notes on why one of the caregivers was unavailable.

Weapons stored in a breakable glass storage did not have trigger locks installed.

It was determined during the course of the investigation that child in care sustained physical injuries due to inappropriate discipline.

It was determined during the course of the investigation that caregivers utilized corporal punishment on a child in care.

Documentation was not obtained for the hospital stay 5.14.21-5.19.21. Along with this, no documentation was provided for the follow up medical appointment that occurred on 6.10.21.

Caregiver left prescription medication accessible to a child in care.

Caregiver admitted to threatening to use corporal punishment on a child in care.

Caregiver left toddler unattended in the bathtub and child sustained an injury during the time that child was left unsupervised.
