It was also determined that one of the two caregivers forcefully picked a child in care up by the arms and placed the child in time-out in a forceful manner.

During the walk-through inspection 3 of the 10 classrooms had diaper creams located in the diaper changing station easily accessible to children in care. This was immediately corrected during the inspection by relocating the diaper creams to a high cabinet where children in care could not reach.

A 3 year old child in care was in the bathroom alone unsupervised by the caregiver.

It was determined throughout the course of the investigation that a caregiver with provisional conditions was left alone with the class for approximately a 2 minute duration.

10 personnel files were reviewed during the unannounced inspection. All 10 personnel files were missing documentation for required annual trainings.

During the unannounced inspection the operation's policies and procedures were reviewed. The operation was missing a Safe Sleep policy. The operation was also missing instructions for contacting the local Child Care Regulation office and the abuse neglect hotline. Specifics regarding physical activity were also missing in the policies and procedures.
