Violation Details
Two training potty chairs in the 2 year old classroom had standing urine in them. The children were observed using the potty chairs without the potty chairs being sanitized between each use. This was corrected at inspection when the caregiver emptied and sanitized the potty chairs.

In the 3 year old classroom, urine was observed on the floor in the restroom. Furthermore in the 2 year old classroom a trash can with diapers was observed without a lid, making dirty diapers accessible to children. This was corrected at inspection when the urine was cleaned off the floor and the floor was sanitized as well as replacing the uncovered trash can with a trash can that has a lid.

During the inspection two playyards were observed with loose fitting sheets while the infants were sleeping. This was corrected during the inspection by placing tight fitted sheets on the base of the playyard.

During the inspection an infant was observed sleeping in the playyard with a bib secured to their neck. This was immediately corrected during the inspection by removing the bib.
It was determined throughout the course of the HHS investigation that caregivers were aware of a caregiver violating minimum standards on multiple occasions but failed to report this to Child Care Regulation.
It was determined throughout the course of the HHS investigation that a caregiver had her back to young children in care during a transitory period towards the bathroom. Surveillance footage also showed 3 children in care in the hallway by themselves unsupervised by the caregiver in charge as she was around the corner and in the bathroom.
It was determined throughout the course of the HHS investigation that a caregiver forcefully grabbed a child's chin with both hands in an effort to get the child to listen. This occurred on multiple occasions. Surveillance footage also revealed the same caregiver forcefully grabbed a child in care by the arm and "flung" the child towards a wall when the caregiver was frustrated. Furthermore, the same caregiver grabbed and pulled other children in care when frustrated, when children wouldn't be quiet, when children wouldn't listen, and during naptime.
It was determined throughout the course of the HHS investigation that a caregiver yelled at children in care on more than one occasion. The caregiver yelled when the children in care were not quiet, didn't listen, or when the caregiver was frustrated.
During the inspection multiple ceiling tiles had water damage. Two fence posts were broken on the playground.
A survey of 10 children records were reviewed. One child in care out of 10 was missing their immunization record. One child in care out of 10 was missing their health statement.
During the HHS investigation it was determined that an infant's head was covered during naptime for 1 hour and 53 minutes.
It was determined throughout the course of the HHS investigation that the infant sheets for a child in care were incomplete on 8/22/22, 8/24/22, and 8/25/22.

Throughout the course of the HHS investigation, it was determined that altercations have occurred between operational staff and the director where yelling and profanity were used while children in care were present. It was also determined during an altercation that the director grabbed the caregivers clothing during a verbal altercation while children in care were present.
It was determined throughout the course of the HHS Investigation that a caregiver was observed by a child in care smoking on the premises in the parking lot.
During the unannounced inspection it was determined that two individuals who periodically drive the transportation bus and have supervised direct care with children do not have background checks.
During the unannounced inspection the infant sheets were dirty with drool stains and hair. The 2 year old classroom potty chairs also had urine in them. The playground had one tripping hazard observed where the children in care play. The sheets were changed and the urine in the potty chairs was emptied and the potty chair was sanitized, placing the operation back into compliance.
During the inspection records pertaining to transportation training could not be located for two individuals operating and driving the transportation bus.
During the inspection the length of the fire drill wasn't documented. The required monthly documentation checks for the carbon monoxide detector, smoke detectors, and fire extinguishers were also not documented.
A caregiver was observed caring for 3 infants alone, violating the provisional status set forth in the background check guidelines. This was corrected at inspection by removing the caregiver from the room and placing the caregiver in another room with another caregiver who met background check qualifications.
It was observed that fire drills have not been documented since May 2021.
A caregiver failed to wash a child's hands after diaper changing. The caregiver admitted to not having water in the basin located under the sink and that was the reason the child's hands did not get washed.
Based on information obtained from a DFPS investigation, a child in care bit multiple children 21 times in a 3 month time period.
