Staff at the operation were using diaper rash cream and Vaseline during diaper changing without written authorization from the parents. The Director instructed all staff to remove all diaper rash cream and Vaseline, and to discontinue usage of diaper rash cream and Vaseline until the parents have provided written authorization.
Infants in the operation that are receiving formula do not have current feeding instructions.
3 staff's files were reviewed, and the annual training information for the current training year was missing.
The operation electrical outlets where children 4 years and younger can reach were not covered.
There were sippy cups and bottles in use for children 2 years and younger, that did not have the child's first name and initial of the child's last name on them. The staff added first name and last name to all the sippy cups and bottles.
Cots in use did not have a labeling (name to cot) system in place to identify a specific cot to the child that uses them. During the inspection, the staff in each class labeled each cot with numbers and created a list with the numbers and the child's name that will use each cot.
A caregiver did not properly account for all of the children in their care when a child that was in their care was left unsupervised on the playground for a lengthy period of time.
