On the school age playground two plastic black borders were observed broken/cracked.

Two of the ten staff files were missing CPR/First aid training.

The operational policy did not specify the indoor and outdoor physical activity, gang free, Immunization requirements for children, hearing and vision screening.

One of the ten staff files were missing 11.50 required training hours.

The mulch on the school age playground was observed to be compact at the exits, entrances and use zone.

Three of the ten children files didn't have current immunization records available.

A situation that placed a child at risk was not reported to the parent for over three hours.

A child was left unsupervised for an extended amount of time.

Based on the evidence gathered, it was determined a child was left on the playground unsupervised.

Two out of ten children's records evaluated were missing the required healthcare professional's statement.

The previous fire drill was conducted on 07/15/22 which is past the required monthly timeframe. This was corrected at inspection when the operation conducted a fire drill.

There were zero of the required food allergy notifications posted in the classrooms regarding children with diagnosed food allergies. This was corrected at inspection when the food allergy notifications were posted in the classrooms.

A person was observed in a classroom responsible for children prior to the operation receiving the eligibility letter stating person could be present at the operation. NOTE: This was corrected during inspection by person leaving the operation.
