This standard was found deficient as part of a DFPS Investigation.

This standard was found deficient during a DFPS investigation when a caregiver grabbed and pulled three different children. One child was grabbed by the arm and dragged approx. 5 feet. One child was aggressively grabbed and pulled by the arm to a sit-down position. One child was yanked up to a standing position.

Classroom was observed with 10 children ranging in ages from 16 months to 21 months with one caregiver. Corrected at inspection when second caregiver was added to classroom.

A caregiver did not wash hands after diaper children.

On 5/2/2024, an employee was viewed on their personal cell phone for approximately 30 minutes during naptime supervision.

Children's hands in a one year old classroom are not being washed after diapering.

The operation failed to do a check for possible hazards before allowing children outside to play on the temporary playground.

The governing body has failed to ensure the operation is maintaining compliance and that the director is completing all required director responsibilities.

A caregiver used poor judgement when the lifting a child from the floor. The method the caregiver used resulted in an injury to the child's right elbow.

The operation failed to report two cases of COVID 19 which occurred between 12/17/2023 and 12/22/2023.

The director used poor judgment when she allowed a staff's child to remain in care with a fever.

The operation failed to follow their Illness and Exclusion policy for children with a fever and allowed a child to remain in care.

The operation did not document fire drills for the months of July, August and September 2023.

The opeation did no thave a documenation showing they have a current Health Inspection.

The opeation did not have a current Consumer Product Safety Commision Affidavit.

One infant's written feeding schedule was not signed by the parent.

The opertaion did not have current documenation showing their annual City Fire Inspection.

A caregiver failed to properly supervise a child when being unaware of the child's activities and not being able to intervene to protect the child's safety. This resulted in the child falling through a railing and sustained a fractured wrist. The caregiver also failed to immediately tend to the child after the child had fallen.

A caregiver did not wash their hands after diapering. The vioaltion was corrected at the time of inspetion.

Staff stated they did not have 12 children wash their hands prior to being served lunch.

One interior classroom did not have adequate lighting during naptime. The violation was corrected during the inspeciton.

The ten files reviewed did not have the admissions form section regarding Emergency Medical Authorization information completed and signecd the parent. 12/9/2022 - This violation has been re-cited.

Six infant feeding schedules are not signed and dated by the parent.
