In the 3 year old classroom children were observed to be running around in the room and the caregiver did not redirect behavior. 1 child observed slapping another child. Children observed climbing furniture and hiding underneath. Children dropped food and ate the food off the floor and one child was observed taking out an electric safety plug from an outlet and attempted to stick his finger inside. One child was observed feeding an infant child food that was on the floor.

The outdoor playground safety checklist was not available for review.

At arrival time, the three year old classroom was observed to have infants and children with more than a 18 month gap in age. The youngest child was observed to be 4 months old and the oldest was 4 years of age.

In the infant classroom, chemicals were observed to be within reach to children at the sink they use to wash their hands. In the toddler classroom, a radio with a cord plugged in the outlet was observed to be with in reach of the children and is a pull/fall hazard.

Two out of the six staff records monitored did not have current first aid and cpr training.

Three out of the six staff records reviewed, did not have the required annual safety trainings.

At arrival time, the opening ratio was observed to exceed the ratio limit and includ infants in care for more than an hour.

One medication monitored did not have the original container.

One out of the six children's records monitored did not have an immunization record.

The first Aid Kit was missing Scissors.

Water was not made available while the chidlren were outside playing.

The Pre-Kinder children do not have age appropriate outdoor play equipment.

Expired Neosporin was in the First Aid Kit. This was corrected when removed from the first Aid Kit.

From January 1st 2024 until May 29th 2024, fire drills have not been conduced monthly.

Shelter in Place/ Sever Weather Drils or Lock-Down Drills have not been conducted.

The annual fire inspection on file expired on April 8, 2022. The operation did not submit the required fire inspection by the required compliance date. This is a re-cite.

One of the 3 children's files reviewed, one did not have the Providers Health Statement section of the Admissions Form completed.

Of the 3 children's files reviewed, two did not have this section completely filled out with the required information. Either the name, address and or phone number was not listed.

Name and address of the alternate shelter away from the center is not included in the Emergency Preparedness Plan. This was corrected when the director added the name and address of the shelter to the relocation alternate shelter plan.

Cots in two classrooms had numbers, however, operation did not have a number/child assignment map available.

One staff did not have an affidavit and has been employed since October 2021.

Three staff did not have current first aid/CPR training. One did not have any first aid/CPR training. Two staff's certificates expired June 18, 2021.

Five of six children files reviewed did not have documentation of any special needs. The statement was left blank.
