Caregiver did not use good judgement when she did not follow the operation's procedures, and as a result a child suffered an injury requiring medical treatment.
A review of the employees who left employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
It was found that a fire drill was falsely documented, when the operation has already entered the information for November.
The storgage container in the Toddler playarea did not have hinged lids. This was corrected during the inpection. The staff removed the storage container and placed it in alleyway on the other side of the fence.
The Infant room had five babies with one caregiver. The Infant room was out of Ratio.
Caregiver did not know how many children she was responsibility for.
The Director did not ensure the Infant room ratio was maintained.
Toys were observed in several of the cribs while the Infants were in the crib. Items were removed when discussed with the owner. This was corrected during the inspection.
Both First Aid kits in the operation were missing the Thermometer.
Of the 4 children's files reviewed, one did not have the immunizations record available for review. This was corrected bring the inspection.
During the observation in the Infant room, eight of the Infants were crying while one caregiver was documenting and the other holding an Infant.
While in the Infant room a baby bottle with formula was observed in the crib with an Infant.

Center was operating without Liability Insurance and did not notify the parent in writing.
The teacher in one classroom had walked out of the room leaving the children without supervision. In less than a minute, the caregiver returned. The correction was made during the inspection.
