The Childrens Courtyard

This standard was evaluated as part of the investigation and was found be deficient. It was determined that on 10/28/2024, one child did not have their soiled pull up diaper promptly changed.

During the inspection, two children were in a restroom in a classroom; no caregivers were in the classroom.

On an outdoor playground on the second level of a slide structure, there was a plexiglass window that was cracked which caused a pinching hazard to the children.

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with caregivers, it was determined that a caregiver is putting a child in a highchair as a form of discipline.

One of the eight personnel files reviewed was missing pre-service training in caring for children under 24 months.

Three of the thirteen children being transported had missing emergency medical transport and treatment authorization forms in the binder the operation takes on the vehicle when transporting children. This was corrected at inspection when the operation added the information for the three children to their transportation binder.

Two of the eight personnel reviewed were missing orientation training in handling, storing and disposing of hazardous materials.

Three of the thirteen children being transported were missing parent's names and telephone numbers and emergency telephone number in the binder the operation takes on the vehicle when transporting children. This was corrected at inspection when the operation added the information for the three children to their transportation binder.

One of the three drivers was missing current transportation training.

Two of the ten children records reviewed were missing immunization records.

Two of the eight personnel records reviewed were missing orientation training in handling emergencies.

Two of the eight personnel files reviewed were missing orientation training in preventing and responding to food allergies.

The operation conducted one severe weather drill in the past 12 months.

The operation conducted one lock down drill in the past twelve months.

The last fire drill conducted at the operation was in January 2024.

Five infants were in the infant room, ages 7 months to 18 months. This was corrected at inspection when the oldest infant, 18 months old, was moved to the two year old classroom.

The loose fill surfacing on the Pre-K/School age playground is less than nine inches deep, and has a structure surface height of greater than five feet.

At the time of inspection, one child did not have their most recent, according to their age, immunization record on file for review.

At the time of inspection, two children did not have the required health statements on file for review.

The infants in care that were still receiving formula and/or breastmilk did not have a current infant feeding instruction form on file for review.

At the time of inspection, the center had not conducted a severe weather or lockdown drill while the children were in care.
