This standard was evaluated as part of the investigation and was found be deficient. It was determined that on 10/28/2024, one child did not have their soiled pull up diaper promptly changed.
During the inspection, two children were in a restroom in a classroom; no caregivers were in the classroom.
On an outdoor playground on the second level of a slide structure, there was a plexiglass window that was cracked which caused a pinching hazard to the children.
This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with caregivers, it was determined that a caregiver is putting a child in a highchair as a form of discipline.
One of the eight personnel files reviewed was missing pre-service training in caring for children under 24 months.
Three of the thirteen children being transported had missing emergency medical transport and treatment authorization forms in the binder the operation takes on the vehicle when transporting children. This was corrected at inspection when the operation added the information for the three children to their transportation binder.
Two of the eight personnel reviewed were missing orientation training in handling, storing and disposing of hazardous materials.
Three of the thirteen children being transported were missing parent's names and telephone numbers and emergency telephone number in the binder the operation takes on the vehicle when transporting children. This was corrected at inspection when the operation added the information for the three children to their transportation binder.
