Operational policies were reviewed and missing the following: (18)(F) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play. (30) Procedures for supporting inclusive services to children with special care needs.

Seven staff files were reviewed. One file is missing notarized affidavit and a second file is missing the translation / equivalence of a foreign diploma.

Most recent health inspection report was not available for review at inspection. Per provider an inspection was conducted 4/2022 but could not be located.

Transportation binder taken during routes is missing Emergency medical transport and treatment authorization forms for each child being transported. Note: provider made copies of the 1st and 2nd page of the admission form and generated an updated transportation binder.

Three out of six staff trainings records reviewed are missing trainings on the following topics: -Safe Sleep / Shaken Baby -Hazardous Materials -Administering Medications -Building and Physical Premises Safety

Outdoor restroom ceiling has build-up humidity.

One out of five staff files reviewed is missing a copy of high school diploma/ged or equivalent.

One out of the eight children's files reviewed is missing a health statement.

The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.
