The toddler playground still has a rake on the ground within the active play space. Additionally, glass cleaner is on a shelf accessible to children leading out to the toddler playground. The rake was removed from the outdoor activity space. The primary hallway used by children has a mop bucket with standing water. The bucket of water was removed.

The toddler classroom has an outlet that requires two outlet covers. Covers were added to the outlet at inspection. Cleaning supplies was on a cart and accessible to children. The cleaning supplies was placed on a higher shelf when requested by child care regulation. A closed used by children for severe weather drills also has cleaning supplies in reach. The toddler playground has a rake on the ground within the active play space.

The required documentation for animals at the childcare center have not been obtained for a dog that is at the operation and in classroom around children. The documentation includes a health statement from a veterinarian and vaccination records.
Based on the evidence obtained during the investigation, the operation has had a dog belonging to a staff member at the operation without parents being notified prior. The operational policies additionally state that parents would be notified prior to animals being at the operation.
An employee was observe at inspection changing consecutive diaper changes and did not wash their hands in between at the hand washing sink.
Two children's records indicated a food allergy on the enrollment record but did not have a complete food allergy plan on file.
The director did not have transportation training available for review. The last one available for review in file was dated for 6/15/2021 and this is required to be obtained annually by the director of an operation who provides transportation and prior to providing transportation to children by staff.
The infant classroom had cheese and crackers for snack and were not provided a plate or napkin for their food to rest on. The food was emptied from the pre packaging onto the bare table. The toddler classroom did have their snack on paper towels.
The hand washing sink in the infant classroom was observed being used to clean pacifiers as well as prepare bottles.
There were various areas of the operation that require maintenance, including but not limited to broken outlets covers with sharp edges, soap dispenser cover that is broken, toy kitchenette that has a broken piece near the sink with a sharp edge, and a pencil sharpener that is only partially secured to the wall with an exposed screw protruding at eye level and the cover to the pencil sharpener is not secured over the grinding mechanism of the pencil sharpener. All identified hazards were fixed at inspection. The two outlet covers were repaired with clear tape so that the sharp edge is not exposed. The sharp edge on the play kitchenette was also covered with tape to prevent the sharp edge from being a risk. The pencil sharpener was removed from the wall. The soap dispenser cover was replaced.
Each infants daily sheets was incomplete and lacked any information about the amount of food or bottles consumed. The information was not added on several of the daily sheets prior to pick up by the parents.

The contracted caregiver counted in ratio received one hour of child abuse training in recognizing, reporting, and preventing abuse. However annual training that is still lacking includes health and safety training in emergency preparedness; preventing and controlling the spread of communicable diseases, including immunizations; administering medication, if applicable, preventing and responding to emergencies due to food or an allergic reaction; understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and handling, storing, and disposing of hazardous materials.

There were hard pieces of plastic from the broken toys that is scattered across the playground that present a choking hazard. It was corrected at inspection with the tricycles and pieces of plastic being removed.
Two mattresses in the infant room are torn that exposes the foam of the mattress and is no longer waterproof.
Two caregivers that have been employed for more than 90 days do not have current training in CPR and First Aid Training. One had training that expired on 12/4/21 and one had training that expired on 8/22/21.

The infant and toddler classroom both have outlets that are not covered. And an outlet in the hallway is also missing an outlet cover.

Based on the evidence obtained during the investigation, multiple bathrooms have not been appropriately cleaned and maintained. Soap dispensers are broken with exposed soap bags. The hand washing sinks and light switches have visible dirt and residue. In addition to the restrooms not being properly cleaned and maintained, the toddler toys on the outdoor have layers of dirt and cobwebs.
The grassy outdoor space in front of the daycare was used for sprinkler activities during the summer with children under the age of 5 and school age children. The area is not fenced in.
Based on the evidence obtained during the investigation, no staff accompany the children to use a restroom in the facility as staff often remain on the playground when allowing children into the building to use a neighboring classroom's bathroom without adequate supervision. This includes not being within physical proximity or being able to hear or see the children they are responsible for.
