Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for when the child was left behind outside in the playground for less than 3 minutes. Additionally, the child was discovered by another individual followed by the director and then the staff were made aware that the child had been left behind and was not with the group of children they were responsible for.

A child was left unsupervised in the playground area without any staff supervision and the primary caregivers were not aware of the child's ongoing activities and did not have visual or auditory awareness.
