A volunteer had access to children in care before having an eligibility notification from the centralized background check unit. The volunteer left the operation during the inspection.

A frequent visitor was present at the operation for at least 3 days in the last 30 days prior to having an eligibility notification received by the operation. The frequent visitor left the operation during the inspection.

Multiple classrooms were lacking lids to trash cans that have dirty diapers and food debri from afternoon snack time. One classroom had a cord hanging from the shelf and is in reach of children and poses a risk. One play house on the playground has a broken piece that is a sharp edge and presents a risk.

For the months of September 2023 and October 2023, there were no documented fire drills conducted.

Four booster seats and one five point harness car seat used to transport children in care were past their expiration date.

Two classrooms were observed with children eating snack on the bare table without a napkin or plate.

Diaper changing was observed in an infant classroom and the diaper changing surface was not sanitized.

One classroom was observed with the older infant not having their hands washed after a diaper change. Additionally, the staff member did not wash their own hands after changing a diaper as well.

Based on the evidence obtained during the investigation, a child was released to an individual that was not authorized to pick that child up at that time.

Based on the evidence obtained during the investigation, the operation did not obtain a copy of an individual's identification until after the child had been inappropriately released to this person and returned.

Based on the evidence obtained during the investigation, an incident report was not completed regarding a situation that placed a child at risk.

An infant was observed in a restrictive device at inspection for more than 15 minutes.

A caregiver was not using positive guidance when communicating with children about inappropriate behavior. They were additionally raising their voice and using a harsh tone.

Climbing equipment that requires 6 inches of loose fill within the use zone was limited and did not meet the 6 inches requirements. Various areas were also compacted down.

Children returned from outdoor activities and were not served water. This was corrected at inspection with director directing the staff to serve water.

Two personnel records was lacking documentation of pre service training. One record was lacking documentation of orientation. Two were lacking the check boxes being selected for orientation and the trainer signature. One record was lacking a copy of photo identification. Two records was lacking a notorized affavit of employment.

Children less than 2 years of age were observed on equipment that is higher than 32 inches at the highest designated point.

The operation failed to notify child care regulation about an arrest of a subject with a background check under the operation within two days of the arrest.

Children were observed at inspection eating snackwith their food served on the table surface without a plate or sanitary napkin. This was corrected at inspection with napkins being provided for the remainder of the children's snack time.

The director and the caregiver that provides transportation does not have current transportation training in their most complete training year or their current training year.

An additional building was added and started being used on May 27th, 2022 without notification to child care regulation.
