Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for after the rest of the class was taken in from outside. The child was left alone, outside on the playground, for approximately 10-15 minutes. Additionally, the child was discovered by another church staff employee who made the operation aware that the child was missing and was outside unsupervised.
