Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for after the rest of the class was taken in from outside. The child was left alone, outside on the playground, for approximately 10-15 minutes. Additionally, the child was discovered by another church staff employee who made the operation aware that the child was missing and was outside unsupervised.

Based on evidence obtained, the operation did not notify parents immediately of a situation that placed their child at risk when their child was left outside for approximately 10-15 minutes without a caregiver present. They notified the parents approximately 3 hours after the incident occurred where they should have been notified once the child was safe.

Children's files were reviewed and there were seven children that did not have the emergency medical authorization complete in file. Three children's files were missing information for the emergency contact listed and one child was missing phone numbers for the people of release provided by the parent. Three children's files did not have the required parent signature on the child admission form.
