It showed that a caregiver did use her hand to hit a child on his mouth with a force strong enough to make the child?s head went backward.

The gate in the back playgound, on the west side of the facility is not secure. The gate has a gap that a child could fit through to the unsecured parking lot. On the east side next to the fence there is a large area with multiple tripping hazards. Boarding that are and the sidewalk where the children play the wooden railroad tie is not secured to the ground, exposing rebar that could cause injury.
