It was observed that the floor in the schooler and the two year old class are dirty with grease, dry leaves, water spilled on the floor from lunch. It was also observed that broken and dirty bucket that contain toys in the two year old room.

The center has not paid the December 2024 invoice for background check.

It was observed that there was not enough loose-fill materials on the school age playground. The mulch was almost gone at the bottom of the climbing equipment. This was cited two weeks ago.

It was observed that there four uncovered electrical outlets in the pre k - pre school room.

It was observed that trash bin in the Pre K - Pre school room and school age room did not have a lid and there are trash inside the bin.

It was observed that there was an Igoloo cooler covered with dirty and has dirty water inside. This cooler was right outside the playground where children in the Two's and Three's year old are playing.

Affidavit for employment was not available for review on three caregivers.

It was observed that children were outside playing with dirty cups and bows that flew out of an uncovered trash bin. This was observed when the Two's and Three's were outside on the playground. It was also observed an uncovered trash bin full of trash outside near the school age playground. This trash bin was accessible to children.

The director and one caregiver have expired CPR and First Aid. Two other caregivers did not have a copy of current CPR and First Aid on file during inspection.

One pre school child was missing the health statement.

Four pre school children were missing copy of most current immunization record.
