The caregiver is still lacking 10 hours of annual training for the 2019/2020 training year, and 17.5 hours for the 2020/2021 training year. The provider must have 30 hours of training each year. The provider must submit 14 clock hours by June 14, 2021 and the reminding 14 clock hours by July 5, 2021.

One of the four children present during the last inspection still does not have complete admission information on file. Of the four records reviewed, the following items are still lacking: - One was lacking the child s address - Two were lacking the date of admission - Two were lacking the emergency contact person - The one schooler was lacking the school age information - One was lacking the doctor s information - The one schooler was lacking permission to ride the bus, etc - Two were lacking signed documentation showing the family received the policies - One was lacking the doctor s statement

The operation did not have a written operational policy.

Hazards were observed in the following areas: Restroom - cleaning supplies and the top of the toilet tank was off. Back yard - equipment, tree saw, etc Back kitchen that leads to the back yard - tools

The first Aid kit did not have a thermometer.

The operation did not have documentation of any fire, severe weather, or lock down drills for 2020 or 2021. The last documentation the provider has was for 2015.

Neither household member had documentation showing completed orientation.

The fire extinguisher in the home was empty.

Two children were observed in baby walkers during the inspection.
