A child's medication was not stored appropriately at the time of the inspection.

During the course of this investigation, it was determined that the child had a history of suicide behavior and risk for harming others but was able to gain access to a sharp object from an unlocked kitchen drawer.

A caregiver allowed 2 children in care to be babysat by a 16- and 17-year-old child without approval or documentation.

A caregiver did not have auditory or visual supervision of 2 children in care for at least 45 minutes; during which time an unannounced visitor came to the home, was allowed in the home and spoke privately with the children.

Foster parent did not follow medical recommendations regarding taking the child in care to follow up appointment to address cough prior to inspection. However, foster parent took child to the ER to be evaluated following RCCR visit. Therefore, this standard will be marked deficient but also corrected at inspection.

A background check was not completed for a family member that frequently visits the home.

A foster home only had 1 unannounced supervisory visit conducted within a year.

The supervisory visit form did not document the names of all the household members present for that visit.

Caregivers were ordered on July 14, 2022, to return a child to the care of the biological parent and/or DFPS. The caregivers did not follow the court order. The CPA did not facilitate the return of the child.

The agency closed the home on 11/4/2022, however, the background check was not invalidated until 8/6/2024

There was no documentation of the discussions about the disaster and emergency plans and procedures.

The disaster and emergency plan did not document the specific evacuation plan for children requiring assistance.

This standard was found deficient as part of a DFPS Investigation.

A caregiver threatened and hit a child in care.

The caregiver did not implement or follow the behavior management techniques documented in the child's service plan or the safety plan documented in the service plan for a child's high-risk behaviors in regard to suicidal ideations.

A caregiver used profanity and belittling remarks towards a child in care as form of discipline the home.

Caregivers were using scratching, pinching, biting, pulling hair, and grabbing the children in care by the neck and their mouth as a form of discipline.

This standard was found deficient as part of a DFPS Investigation.

A child under the age of six was bitten in the face by a dog and was left unattended on a countertop.

The March medication log was not being filled out after medication was distributed

One of the child records did not have a date the service plan was completed.

Two staff records did not have a signed statement about reporting any suspected abuse and/or neglect.

The supervision requirements were not followed per the service plan for a child in care when they were left home alone for an hour.

Video cameras are used in the child's bedroom without consent from the conservatorship worker.

The child did not have the correct amount of medication at school which resulted in the child receiving a partial dose. Emails/calls to the foster parent were made but the medication was not delivered timely by the foster parent which resulted in the child only getting 1 pill instead of 1-1/2.

The child in care continues to sleep with the caregiver. However, the variance expired on 12/31/2023.

There were two foster parents that had not renewed their medication training within the allotted time that was required.

Certificates that were reviewed indicated that a foster parent was late in renewing their trauma training.

There were several service plans that were reviewed that were not filled out completely or had N/A written on the form.

A background check was not completed for an extended family member that frequently visits the home.

Foster parent confirmed leaving the children in the car while in the garage, when the children in care had fallen asleep in the car on the drive home.

Both caregivers were yelling at children in care as a form of disciplinary measures.

Caregiver did not provide appropriate supervision which allowed a child to gain access to an iron resulting in the child being burned on the face.

Both child files were missing required documents in their records.

A foster home has not had a supervisory visit conducted with both foster parents in over 6 months.

A child was being "popped" on the hand by a caregiver.

Durig the investigation, it was discovered that there is a frequent visitor who does not have a background check.

The agency did not report a serious incident to the hotline when an infant was left home alone.

A 13 month old infant was left alone in a foster home for an unknown amount of time, ranging from 5 - 15 minutes.

An infant was placed on a bed to sleep with pillows and blankets around them.

Foster parent allowed a young child to use a treadmill for exercise and as a result the child fell and sustained a few scrapes.

The caregivers physically abused 7- and 9-year-old children in care.

There was no evaluation or corrective action taken after the foster home had 2 previous investigations with similar allegations.

The caregivers used physical discipline with children in care including: spanking, hitting, grabbing and pulling hair.

The caregiver yelled at the children in care.

A child in care was forced to eat food.

The service plan for a child stated they were not to be left alone with any other child in the home. This was not followed.

The foster parent used inappropriate language when speaking with the children in care that would be considered sarcastic.

A relative of the foster parent had been staying in the home several nights a week and is a frequent visitor. The relative did not have an approved background check.

A foster home did not conduct the required annual severe weather drill.

The agency did not have a continual evaluation of the unauthorized absences for the last 5 years.

Foster parent confirmed children in care are not allowed in their bedrooms other than to sleep or change.

A caregiver was not close enough to intervene when a child in care was hit with a baseball bat by another child.

A background check was not completed for an extended family member that frequently visits the home and is also a babysitter for a child in care.

Foster parents allowed a person with an ineligible background check to be present with children in care.

A person with an ineligible background check visits the home frequently.

A child in care did not receive 2 of their medications in the morning as prescribed.

A caregiver threatened to whoop a child in care.

The notification for the initial service plan was not sent timely to the foster parents or case worker.

The home screening mentioned they had $4400 dollars in monthly income, but the documentation did not reflect that.

The CPR/First Aid, Normalcy, and EBI training were all expired before the home was closed.

A child in care left the foster home without permission twice. A safety plan for high-risk behaviors was not implemented by the operation.

Foster parents were not provided a copy of the agency's Professional Parent Agreement.

The foster parents did not implement the service plan by failing to make proper notifications regarding travel and trips

Two children reported being yelled at by a caregiver. Additionally, two children reported being cursed at by a caregiver.

During an unannounced home visit 3 medications were observed unlocked on the master bathroom countertop.

The medication log was not filled out for October.

A caregiver brushed a child's teeth with soap.

The back door does not have a lock that a child under 12 cannot reach. There is a door alarm on the back door.

Caregivers did not get a child in care medical care after they sustained an injury that was later determined to be a fractured clavicle. The child complained of pain and requested pain medication for at least 2 days.

A frequent visitor for the foster home does not have a background check completed.

The fire inspection for the foster home was out of compliance from the months of June 2021 to November 2021. The inspection is now current.

The fire extinguisher expired on May 2021.

A child was hit with a belt by the foster parent as a form of discipline.

The adoptive home screening did not properly assess the resolution of an applicant's experiences of childhood abuse and neglect

Caregiver did not comply with the service plan requirements when the biological parents of a child in care were allowed contact with the child at the foster home. The service plan prohibited contact at the foster home.

The caregiver failed to use prudent judgment by engaging in social activities with the biological parent of a foster child placed in the home. The activities included consuming alcohol and allowing the biological parent to stay overnight in the foster home.

A child was not allowed privacy during phones during therapy or with case worker.

Caregiver and household member are listening to the foster child's private conversations with video cameras placed in the home and the child's bedroom.

The caregiver and household member spanked children in care with a belt.

Household member demanded that licensing take a foster child with them during the inspection of the foster home.

One out of two files reviewed did not have a notarized Licensing Affidavit for Applicants for Employment form.

There was over the counter medication in the master bathroom cabinet that was not locked.

A child was allowed to walk home from the store without adult supervision.

The two fire extinguishers in the home expired on October 2020.

A caregiver did not provide adequate supervision allowing two children to sexually act out.

The foster parent did not provide any addition supervision measures to protect the child.

Both foster parents admitted to yelling at her foster children in care.

Both caregivers would get into an argument in front of the foster children and one caregiver shoved the other caregiver in front of the two children.

The weapon was locked loaded with the ammunition.

The foster parent carried a child to the shower.

The foster parent yelled at the foster child.

Foster home had several stressors, including requests to the agency for respite. Agency did not provide the support services necessary to keep the home safe.

Foster father had major medical issues within the past few months including two ruptured discs and abscess on his spine that were not updated in the home screening.

The foster parents did not take the foster child to four required follow-up medical appointments. Also, there were multiple physicals and speech therapist appointments for the foster child that were re-scheduled, double-booked or canceled.

A caregiver held a gun up to his head and pulled the trigger in response to being angry.

A caregiver put a firearm to his head and pulled the trigger in the foster home while a child was placed in the home.

Three of the supervisory visits did not document the names of all household members present during the visit.

Agency documented the foster father's family issues, but did not indicate methods of responding. Further a child with serious medical needs was placed in the home. The stressors associated with this were not identified or addressed. Further the foster father's medical condition had deteriorated greatly and this was not addressed.

The home screening does not document the interviews with the two adult children of the foster parent.

The caregiver did not provide adequate supervision while a youth was visiting a foster child and engaged in inappropriate behavior.

A 14-year-old was able to engage in sexual activity with another youth while the caregiver was in another room.
