Subsitute caregiver informed that current CPR training was 100% online and did not involve the use of a mankin.

An 11 month old was in a restrictive device (bouncer) over 30 minutes. This was corrected at inspection by removing the infant from the device.

Substitute caregiver's file lacked affidavit for applicants for employment form.

The children products certification form was located in the primary caregivers file in a binder. It was not posted in a publicly accessible place for parents. This was corrected at inspection by placing the new cpsc form with the date of 1/3/2023 over the cpsc form posted with a date of 1/5/2021.

Substitute caregiver served food to youth before hand washing. This was corrected at inspection by completing hand washing.

Policies lacked instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website.

Only 2 children's files had notice of lack of liability insurance.

There is no animal policy regarding the dogs in the home.

The substitute caregiver left an infant on the changing table unattended and the infant fell off the changing table.

The substitute caregiver placed a child in play yard with no mattress. There was cardboard and board of wood under the sheet. The play yard did not have manufactures mattress.

During naptime one infant was sleeping in a pack and play with a bottle.

During nap time the play yards were covered with blankets on the sides blocking inside and outside view.This was corrected at inspection by removing the blankets.

A caregiver turned on the television while supervising children under the age of 24 months.

The operation has a person in the home that has a pending background check. The person has access to the children.

The caregiver was unaware that a child was left unattended in a bathroom. The caregiver did not know the ages of the children that they were supervising. The caregiver did not know the names of the children that they were supervising.

There were two ladies at the home with pending background checks with 8 children in care,

There were two training cups and a bottle seen on the counter not labeled.

The children's product certification verification form was last completed on 1/5/2021.

A first aid kit was seen at the home incomplete. The first aid kit was missing: A pair of gloves, assorted sized bandaids,guaze that are enclosed, antescpetic that is useful ( The antesceptic was dry) , Tape, and a thermometer.

The menu observed at the operation was for infants and was only completed for lunch,pm snack, and super.

There were two people seen at the home without a cleared background check. -One lady left the home at the home at the time of inspection. An administrative penalty in the amount of 100% of the daily maximum, per HRC 42.078(a-2) will be recommended as a result of this citation. An additional penalty in the amount of the daily maximum will be recommended for each day past the Compliance Date that the deficiency is not corrected. It is your responsibility to notify Licensing when you have corrected the deficiency

There were mats seen at the home, but did not have the labels of the children name on the mats.

-The operational policies did not have information on how parents can contact licensing or the abuse and neglect hotline. -The operatinoal policies did not have information on vaccine preventable diseases for the employees.
