During the DFPS investigation, an infant room caregiver stated that she was not allowed to carry any children due to her high-risk pregnancy. The caregiver was left alone in the room to care for the infants while the other caregiver was called to the front door to attend to parents picking up their children. The caregiver was unable to properly care for the injured infant and infants in care due to her health restriction.
It was determined during a DFPS investigation that the caregivers did not notify the parent of the child's injury until 30 minutes after they noticed the child was favoring her arm and a total of 50 minutes after the initial injury.

One of eight children records reviewed did not have names and telephone numbers of persons that the child can be released to on file.

I observed 11 children in the infant room with a specified age group of 0-11 months. The maximum amount of children within this age group is 10. One child had just arrived to the room and another child left approximately 15 minutes later.
