There were uncovered electrical outlets in the purple room behind the toy shelf. Pictures taken.
It was observed that there were spaces in the outside playground fencing. The spaces were more than 4 inches wide (according to Arlington Health Department). These spaces posed a risk to children as their head, arms and legs can go through the space. It was also observed that the fence was not stable, it was loose and will feel down with a push. There were also high weeds on the playground, these weeds are approximately 2 to 3 feet in height. A broken chair also observed on the playground. It was the red chair. It was observed that there are 2 broken tiles in the hall that lead to the kitchen. Pictures taken.
All deficiencies cited by the health department were corrected.
