Based on the information obtained during the DFPS investigation, a lack of supervision was determined. A child was left outside on the playground during the transition back inside the building. The child was sleeping inside of a tunnel on the climbing structure.

Three staff persons were present and had direct access to children prior to the operation receiving the eligibility determination letter. The staff persons were removed from the classrooms and sent home.
The director did not demonstrate good judgment when placing their hand over the child?s mouth to quiet the child. As a result, the child sustained a small scratch on their cheek.
Based on information obtained during an investigation, it was determined that the snack the child is served is based on the child?s behavior. Children that are on ?green face? are given what is called a ?super snack.? Children on ?yellow or red face? are given a different snack.
