It was observed a caregiver with provisional eligible with conditions alone with children. She was observed in a four year old class by herself. She also observed in the two year old class by herself when the other caregiver takes children to the restroom. She's also by herself when she took children to the restroom.

It was observed that there was no light in the bathroom. The light went out since April 19, 2024.

Photo Identification needed for two out of three caregivers present.

CPR and First Aid needed for a caregiver who is in charge of a group of children.

Upon arrival I observed the two year old class has 13 children with one caregiver. This was fixed during inspection by adding another caregiver to the room. I also observed a caregiver left the two year old room to go prepare lunch. Which left one caregiver with 15 children in the room. They're 4 children over child/staff ratio.

A notary affidavit for employment is needed for two out of three caregivers present.

Proof of current liability insurance was not available for review at inspection. Liability insurance expired on 3/04/2024.

The owner/director did not follow the minimum standard rules by not notify Licensing when she relocates children to her residence. Children and caregivers were at her residence for two weeks when the operation is closed due to not paying the rent.

Owner/director stated that she did take children to her residence and care for them for almost two weeks while the operation is closed.

The facility did not have a current fire inspection.

An infant was asleep in the car seat. He was removed when the caregiver was informed that he could not sleep in there.

The carseat the infant was sleeping in was covered by a blanket.

An infant was in care with no records or documentation.

The facility had one infant and 11 preschool age children in care.

The only staff in the building was the person who was caring for the napping children.

An infant was in care at the time of the inspection. The facility is not licensed to care for infants.

The facility did not have the most recent inspection form posted.

Policies read during the inspection did not address several required standards.
