Four children were left unsupervised in a classroom for approximately 5 minutes, after being brought back inside from an outdoor soccer activity.

Of the staff files evaluated, 5 lacked documentation of the full 24 hours of required annual training for their respective training years: One staff lacked 10 hours for April 2023-24. One staff lacked 8 hours for Feb 2023-24. One staff lacked 17 hours for June 2022-23. One staff lacked 20 hours for July 2022-23. One staff lacked 15 hours for Nov 2022-23.

It was determined that caregivers left a child unsupervised on the playground briefly - during the transition back inside to the classroom.
