While doing a walk through of the infant room, 1 child was in a restrictive device inside a crib and 2 infants under 12 months had blankets on them. This was corrected when the caregivers removed the restrictive device and the blankets from the crib while I was present.

During the inspection, a classroom was observed with a child's television program on that was not related to the daily activity. The caregiver stated she turns it on in the morning to start the day and wait for devotion time.
During inspection, 2 infant classrooms were observed to have infants in restrictive devices. A second walk through was conducted 30 minutes later and one class had the same 2 infants in the restrictive devices and the second class had the the same children in restrictive devices and one asleep in a restrictive device. This was corrected when the caregivers removed the infants from the devices.
During inspection it was observed the operation did not have a staff list or licensing inspection posted.
Tiesha Carter is a designated perpetrator of neglectful supervision, case #48871715, date 12/4/21, with conditions. One condition is that Tiesha is unable to supervise children alone. On 4/14/22, Tiesha Carter was observed supervising two children alone during transition to a classroom.
During inspection, two infant rooms were observed to have infant feeding sheets that had not been updated. This was discussed with the caregivers and the person in charge.

Based on a DFPS investigation, it was founded that an employee did not properly utilize the installed electronic child safety alarm. The employee exited the vehicle to turn the alarm off, instead of walking through the vehicle to turn the alarm off.
This standard was evaluated as part of a DFPS investigation and it was determined that neglectful supervision occurred.
Based on a DFPS investigation, an operation employee did not use good judgment when they exited the vehicle without following transportation protocol. As a result, a child was left unattended on the vehicle for over 20 minutes.
