Two children with diagnosed food allergies did not have the medication listed on their food allergy emergency plan. Four food allergy emergency plans were not signed by the child's parent.
One caregiver did not wash an infant's hands after changing their diaper.

Three out of four staff files did not have training in SIDS and Shaken baby syndrome, hazardous materials, administering medication, or food allergies.

While reviewing the operational policies, the following policies were missing, infant safe sleep, transportation, emergency preparedness plan, nursing mother, preventing and responding to abuse and neglect and gang free zone.
While revieiwng ten staff files, four did not have all 24 hours of annual training.
During the inspeciton it was found that the operaiton is not documenting the changing of batteries and testing of the carbon monoxide detectors.
While reviewing records the playground maintenance checklist was not availble for review. The operation also did not have the fire extiguisher checks and the smoke detectors checks docuemtned.
During the walk through in an infant classroom, two bottles of cleaning supplies were in reach of children. This was corrected during the inspection when the supplies were put on top of the cabinet out of reach of children.
There is sufficient evidence that the listed director has not been at the operation since October 2020, due to COVID-19.
