During the inspection children were observed in multiple unsafe situations that could have cause a potential incident or injury.

Based on the inspection, and interviews, it was found children were not properly supervised on the playground. A child was injured and the caregiver was unaware.
The caregivers were unfamiliar with which parent went with which child. There were no checks done to verify the person picking up was a listed pick up.
The operation was out of ratio with one group having 29 children and the other group had 38 children. Each group only had one caregiver.
The caregivers were not sure of the number of children present until 4:37 pm when the children had already been in care for over 40 minutes .
A caregiver was yelling at the children since they were not listening.
A child was not able to be found for several minutes after the parent arrived. The caregiver walked around asking where the child was.
