A caregiver working with infants does not have current training in SIDS.

Based on evidence gathered during the investigation, a caregiver did not have a current CPR/First Aid training.

There were 7 staff present without an eligible background check run through the DFPS system. This was corrected at the inspection when staff without background checks were sent home.

Based on the evidence gathered during the investigation, an infant was left unsupervised in the classroom for around 2 minutes.

Three children were able to get out of a gated play area, without supervision. The director of the operation admitted that the children were unsupervised.
