During the course of a DFPS investigation it was found that a child was neglected.

During the course of a DFPS investigation it was found that a child was able to leave the operation unsupervised and was found outside the building on the walkway by a third party employee.

During the investigation inspection it was found that the inflatable was installed over concrete. This was corrected when the inflatable was removed from the premises.

One of two buses used to transport children did not have a properly working alarm.

Two caregivers did not have in person CPR training.

Based on the information gathered during investigation, it was determined that a caregiver left a child alone in the gym.

A caregiver was changing a child in the bathroom located in the classroom and could not see the other children in the room until they walk completely out the bathroom.

2 out of the 10 caregiver files reviwed did not have up to date CPR and first aid certifications. 1 had an online only certification.

A caregiver in toddler room did not wash child's hands after changing their diaper.

A caregiver did not clean diaper changing area after changing child's diaper.

Information obtained throughout the investigation shows that a caregiver failed to demonstrate good judgement when they blew smoke in a child's face.

Information obtained throughout the investigation shows that a caregiver used a vaporizer in the presence of children.

While reviewing 10 staff files, two were missing emergency preparedness, administering medication, food allergies, building safety and hazardous materials training.

Out of 10 childrens files, one was missing current immunization records. This was corrected when the parent emailed the updated immunization records. These were printed and placed in the file.

During the inspection it was found, one infant did not have current feeding instructions. This was corrected during the inspection when the parent was able to email the completed form to the operation.
