Information obtained throughout the investigation shows that caregivers grab children by one arm when they are getting children down from climbing furniture.

It was found during the inspection there were two classrooms that had missing outlet covers. This was corrected at the inspection when the director covered the outlets that were missing covers.

The operation has started the process of getting a fire inspecction completed, it has not yet been conducted.

One infant classroon was observed to have five infants under 12 months old with one caregiver. This was corrected when one of the infants was moved to another room until the second caregiver arrived.

The operation's last fire inspection expired on 12/17/21. The director stated she has been trying to contact them to schedule the inspection since before it expired.
