At inspection, a child in care that had a known food allergy, did not have an emergency action plan form available to CCR.

During the walk through of the areas the children use it was found that in the gym, there were three lightweight metal shelving units and one metal filing cabinet along the walls of the gym. On the right side wall of the gym there was an unplugged television, leaning against the one wall. Towards the back of the gym there was an unplugged popcorn maker, a battery charging station and a brick, easily accessible to children.

During the investigation inspection it was found that two caregivers were present who were counted in ratio did not have an eligible background check. One caregiver had a group of 13 children and the other had 14 children. Both caregivers left during the inspection, the floater took over a group of children. Then the Program Director was called to the school to take over the other group.
