It was found that a child in care sustained an arm injury on 4/4/24 at approx. 10:00 am while playing at the gym at school, however the child wasn't seen by a doctor until 4/6/24 around noon time in which it was discovered the child had an arm fracture.

It was found that the netting and metal poles had been removed from the trampoline in the home leaving the metal holes with sharp edges exposed. The child in care fell on one of the metal pole holes while on the trampoline and hit their cheek resulting in a cut and received medical care.

During an unannounced investigation, a 16 year old was babysitting a 1 year old child in care and is not CPR certified.

During the invesigation investigation, the medication log was observed to be blank. This was corrected at the inspection when the caregiver completed the medication logs for the child in care.
One of the children in care is not taking their prescription medication as prescribed by the doctor.
Of the 18 serious incidents reviewed four were related to restraints. None of the four reports had all of the required documentation.
Documentation reports that a child in care as well as an adult were diagnosed with Covid-19 and were not reported to the Hotline timely.
One of the four adoption/ foster child files did not have an admissions assessment in the child file.
One of the two foster home did not have a discharge summery for a child who was discharge then came back into care.
One of the four adoption / foster child files evaluated did not have a current immunization records on file.
Several trainings evaluated in all four staff training files did not have the trainings qualifications listed on the certificate.
One of the four child files reviewed had a medication record that did not include the reason why the medication was prescribed.

One of the 8 home filed evaluated had a home that did not have a family reference listed. The other required references were in the file but not a part of the home screening.
A Contact Note Report, dated 07/26/2021, provided by the operation noted that the foster father had been exposed at work to an individual that was positive for COVID-19. Foster father tested for COVID-19 on 07/25/2021 and on 07/26/2021, the test results returned positive. On that same Contact Note Report, it was documented that the operation advised the family to call the Texas Abuse and Neglect Hotline to inform the Department that a member of the home was positive for COVID-19, yet it was not reported until 07/28/2021.
During the walk through medications were found in an unlocked cabinet and on the bathroom sink for the parents in their room.
During the course of the investigation, caregiver admitted to posting about incident on social media.
Caregivers admitted to attempting to get phone from victim child. They admitted to blocking door and preventing child from leaving.
During the course of the investigation, it was determined that the situation was not an emergency and therefore restraints were not warranted.
