10 staff files were reviewed. One Staff Member needed food allergy training. Two Staff members needed training in hazardous materials, building safety, and emergency preparedness.
One classroom was missing an evacuation plan. This was corrected at inspection when the Director printed out a new plan and posted it in the classroom.
The operation had two expired medications on site.
The operation was missing the signed annual recall product certification form. This was corrected during inspection when the recalls were reviewed, the certificate was printed, and it was posted on the parent board.
Caregivers did not use good judgment when deciding to both change diapers at the same time, which allowed a child to be injured as neither caregiver could quickly intervene.
Based on the the information obtained during an investigation conducted by DFPS, it was found that a caregiver demonstrated poor judgment when forcefully placing a child in a crib resulting in an injury.
