While reviewing four children's files, two were missing the address for the emergency contact listed. These were both corrected when the caregiver contact the families and obtained the addresses.
While reviewing the household members file, it was found that the member did not have proof of orientation. The primary caregiver did not have orienetation or a copy of their photo ID. The missing information was corrected and placed in their files during the inspection
During the inspection it was found the animal in the home did not have current vaccinations.
While reviewing the operational policies it was found that realease of children, discipline and guidance, infant safe sleep, animal, the abuse and neglect hotline telephone numbers as well as the emergency prepardness plan were missing.
During the inspection the consumer product safety form was out of date. This was corrected during the inspection.
