A caregiver did not know the number of children she was caring for. The caregiver said their were 8 children present, but then corrected herself and said it was 9 children. The children were counted and there were actually 10 children present in the room.

After sending several reminder emails concerning background check fee payment, as of 6/2/2022, the operation has not paid pass due background check fees.
Supervision was not maintained when two children were able to climb a fence and leave the playground area.
In the preschool class there were two different schedules posted in the classroom. It was observed that one scheduled reflected for the outside time to be at 3:30pm, however it was observed that the children were in activities in the classroom at the time.The other classroom reflected that outside time was at 5:00pm. Caregivers stated they go outside at 4:00pm or whenever classes combine. In the pre-k class the schedule posted reflected outside time at 4:30pm. The children were observed lining up to go outside at 3:30pm. The school-age activity plan was dated for 3/8/3021.
The caregiver in the school age class reported that there were 26 children in care, however there were 25 children present. The caregiver did not know the specified age group for the class and was unable to determine what it was.
The walls throughout the operation have visible dirt and grime on them. The carpets in school age, 2 toddler rooms have visible stains and dirt on them. The cots in the preschool room (15) are visibly covered with dirt all over them and the cots in the Discovery toddler room (13) have visible dirt on the sides and handles.
The toddlers on the playground were climbing on the fence and the caregiver was directing them to climb down by shouting from the side of the building 10-15 feet away. At that time another child was putting rocks in their mouth while another child was pushing the toy care into that child. At the same time 2 children were fighting over the basketball goal. They were pushing one another fighting of the poles of the goal.
The sanitizing spray was left out on the sink area in the child's bathroom. The Assistant director moved it to the closed cabinet in the bathroom, out of the reach of the children.
The pre-k and pre-school playground had 4 used face masks scattered around the playground. There were used and broken cups, popsicle sticks and wet paper towels scattered around the playground. The area leading to the playground had 2 empty mop buckets, trash and broken toys in the area. The Assistant Director and Program Specialist cleaned both areas and removed all hazards.
The caregiver in the Pre-k class stated there were 11 children in her care, however there were 12 children present in the classroom.
The table in the pre-k class was not cleaned after snack. The table had an used empty plate and peaches and syrup were visible across the table. There was empty used napkins and cups on the floor under the table. During the inspection the director cleaned the table and floor.
An outlet was not covered in the Pre-K classroom. The director covered the outlet with a safety cover during the inspection.
