During the investigation, it was discovered three caregivers had CPR/First Aid certificates that were from online-only sources. The caregivers did not do practice on a CPR manikin as part of the training as required.

Based on a DFPS investigation, it was determined that improper discipline was used. A caregiver grabbed the arm of a child and yelled at them.
One classroom was unaware of how many children they had, and one classroom was unaware of the different age groups that were present.
