A child was bitten while in care, and parents were not notified about the bite marks.

There was a lack of supervision when a child was bit 8 times in the classroom.

A current fire inspection was not available for review.

The operation did not have the Keeping Children Safe poster or the Parent Notification poster.

Five caregivers did CPR classes that were fully online. These classes did not include the required practice with a CPR manikin.

The operation did not have a current annual sanitation inspection.

The previous sanitation inspection occurred on 06/11/2021 which is outside of the required annual due date.

The previous fire drill was conducted on 01/27/22 which is outside of the required monthly due date. This was corrected at inspection when a fire drill was conducted.

The previous fire inspection was conducted on 10/28/2020 which is past the required yearly due date.

A child was left outside on the playground, it was determine the operation is in violation of the minimum standard supervision and cited.

The operation has not been reporting positive COIVD cases to Child Care Regulation.

Based on a DFPS investigation, it was determined that improper discipline was used. A caregiver grabbed the arm of a child and yelled at them.
