A crib that an infant was sleeping in had a bottle and a blanket it in with the infant. This was corrected at inspection when the caregiver removed the items from the infants crib.

During the follow-up the liability insurance policy unavailable for review. The most recent policy expired on 1/31/2024. 9/27/2024-There still was no active liability policy available for review. Director stated she sent the notice to inform the parents of no liabilty insurance on the operation back in March 2024.

Neither the director nor the caregiver had CPR and First Aid certification for review.

There were many tripping hazards present in both play areas.

The illumination within the initial classroom appears to be malfunctioning and requires immediate attention.

Trainings were not available during the inspection.

There were several walls and equipment that needed cleaning or replacing. There were dead crickets throughout the building.

During the walktrhough, there were piles of items and clutter causing a tripping hazard and possible health and safety hazareds. The following items were observed and accessible to children: Pile of clothing and blankets. Stacked area of open plastic bins containing clothing and supplies. Some supplies are spilling out of the containers onto the floor covering the walking area. Shelves with a bottle of sode and can or pringles. Chairs, foam tiles and other items on the floor of the changing area. Rugs with visible dirt and trash.

An 8 mo old child was combined in a room with a 3 year old child (exceeding 18 mo age gap).

A garbage can was observed to be full of garbage without a lid, and accessible to children.

A child was observed sleeping in a bouncy seat. This was corrected when the child was removed from the seat.

One classroom was over ratio with one caregiver supervising 10 children ages 8mo-3 years old with a SAG of 18-24mo. This was corrected at inspection when additional caregivers returned from lunch.

An 8 mo old, non walking infant was in a classroom (during naptime) without a crib for sleeping.

During an inspection, children were observed unsupervised during several incidents.
