A caregiver knowingly and regularly leaves children 18 months through three years of age alone in the classroom to take a child to the bathroom across the hall. caregivers allow children to leave the classroom to use a bathroom in the hallway.

A child left the classroom unnoticed by the caregiver.

During an investigation, staff was instructed on what to say and to provide false information to the Child Care Regulation Inspector during interviews.

During the monitoring inspection it was found that the operation did not have the most recent inspection posted.

During the monitoring inspection it was found that the director only had 3/30 of her required training hours.

During the monitoring inspection it was found that the director's CPR and First Aid training had expired in Jan 2024.

During the monitoring inspection it was found that the last documented fire drill was 11/15/2022. This was corrected at inspection when a drill was conducted and documented while CCR was present at the operation.

During the monitoring inspection it was found that the operation had not serviced their fire extinguishers since August 2022.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

During the monitoring inspection it was observed that the toddlers were having their diapers changed on the classroom floor, next to the circle time area. This was corrected at inspection when they started using a unused nap mat for diaper changing and removed it from the play area when they were finished with diaper changes.

During the monitoring inspection it was found that the sanitation inspection had expired 10/2023.

During the monitoring inspection it was observed that there was a play structure on the playground area that had not been approved by licensing. This structure surpassed the maximum height of 32 inches requirements for children under two. It also was not in compliance with swing set structure requirements or use zones areas.

During the monitoring inspection it was observed that the loose fill under high use areas had been dispersed and under the play dome there was an area with less than 1 inch of loose fill.

During the monitoring inspection it was observed that a caregiver was changing a diaper in gloves and did not wash their hands after removing their gloves. This was corrected at inspection when the caregiver was asked to wash their hands between each diaper change, even if wearing gloves, and complied.

During the walk through of the operation, children were observed exiting the second story of the operation. The director was told by the Fire Chief that care could not be provided for children under the age of 5.

The operation did not have a handwashing sink in the diaper change area.

The operational policy was missing the areas explaining employees vaccinations and preventing/responding to abuse/neglect.

A child's Epi-pen expired on 10/6/22. A child's oral medication expired 9/24/22.

Two staff members had CPR training but it was not completed in person using a manikin.

A staff record was missing the signed affidavit.
