Primrose School of Austin at Mueller

One staff member was accidentally inactivated on 2/23/22. They were present at the operation without an updated background check eligibility notification. This was corrected at inspection with the staff member left the operation while I was present.

Updated infant feeding instructions were missing for 8 children. Two of these were corrected during inspection when the parents were contacted and stated that there were no changes.

10 staff files were reviewed. One staff member needs proof of her 2020-2021 trainings, and one staff member needs proof of two more hours of training.

One allergy action plan needs to be signed by both the parent and the doctor. Two allergy action plans need to be signed by the doctor.

Expired medication was located in the medication bin during the inspection. This was corrected at inspection when the expired medications were removed from the medication bin.

One child was missing an up to date immunization record.

5 staff training files were not available for review during the inspection.

Two classrooms had exit doors that were blocked by naptime cots. This was corrected at inspection when the cots were removed from in front of the doors.

Drill logs have not been documented since August 2021.

Two staff members need notarized affidavits for the HR file.

During naptime one classroom was out of ratio. There were 10 children in care with one staff member. There were children in the room under 18 months old. This was corrected at inspection when the second caregiver returned to the classroom.

Two staff members were unaware of the number of children in their classroom.

A copy of the updated annual fire inspection was not on file at the operation.

Three caregivers were missing the notarized affidavit in their staff file.

One caregiver was missing a copy of their diploma or transcripts in their staff file.

10 staff files were reviewed. Six caregivers were missing training in child maltreatment.

One caregiver needed to renew their CPR/First Aid training.

Two classrooms were unsure of how many children they had and had to count. One classroom was unsure of the ages of the children.

There is sufficient evidence that a caregiver sexually abused a four year old child.
