Based on information gathered during the course of the investigation, it was determined that a caregiver inappropriately handled a child while in care.

Based on information gathered during the course of the investigation, it is determined that caregivers in 1B, 2A, and 3B have been observed yelling in the children?s faces and using aggressive language. It is determined that the caregiver in 3B has used other children to taunt and humiliate children who wet themselves, misbehave, etc.

Based on information gathered, no incident report was created for an incident involving a child being placed out the back door of the classroom and being left outside with no direct supervision. The incident reported was not created as of 6/25/2025.

Based on information gathered, it is determined that an infant/toddler caregiver has frequently not promptly changed children?s diapers resulting in diaper rashes and dried poop being found on them by other caregivers.

Based on information gathered, no incident report was created for an incident involving a child being placed out the back door of the classroom and being left outside with no direct supervision. The incident reported was not created as of 6/25/2025.

Based on information gathered during the investigation, the operation has two subjects with active background checks who have not been employed there for over 7 days.

Based on information gathered during the course of the investigation, it is determined that caregivers in the operation?s 1B, 2A, 2C, and 3B classrooms have been witnessed inappropriately grabbing and pulling children. One caregiver was observed pulling a child up off the floor by the collar of their shirt.

Based on information gathered during the course of the investigation, it is determined that approximately since May of 2024, the director and assistant director have been knowledgeable of multiple ongoing concerns of minimum standards violations at the operation that places children at risk.

Based on information gathered during the course of the investigation, it is determined that caregivers in the operation?s 1B, 2A, 2C, and 3B classrooms have been witnessed inappropriately grabbing and pulling children. One caregiver was observed pulling a child up off the floor by the collar of their shirt.

Based on information gathered during the course of the investigation, it is determined that the operation has allowed children with a fever to remain in care. When children are sent home with a fever, they are not required to remain out of care until 24 hours symptom-free.
