Baytown Early Headstart and Headstart Center

I reviewed 10 children's records, and it was observed that two children physician information was not documented on the student emergency contact information or admissions forms.

During the inspection the fire inspection on file was dated 10/28/22. The operation was able to provide a request submitted on 04/14/23 which is still out of date.

During the inspection signed copies of parent receipt of operational polices and/or parent handbook were not available for review.

Documentation of the annual fire inspection report was not available for review.

A caregiver threatened to pop children on two different occasions.

The operation did not enter the number of employees who left the operation in 2023. This task was due no later than January 15, 2024.

Based on information provided as a result of children being inappropriately touched it is determined caregivers did not provide proper supervision.

A caregiver does not have CPR/First Aid training.

Based on information obtained during the investigation inspection and interviews, there is sufficient evidence to support the allegation of inappropriate discipline.

Based on information received during the investigation inspection, there is sufficient evidence support the allegation of the child being inappropriately disciplined.

Staff did not have annual training hours in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medication, and Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing and controlling spread of communicable diseases and Building and Physical premises safety.

The director did not have annual training hours in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medication, and Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing and controlling spread of communicable diseases and Building and Physical premises safety.

The operation has not had an annual fire inspection conducted since 8/7/2020.
