Three caregivers were missing required annual trainings.

The first aid kit was missing cotton balls, scissors, and tape. Note: Corrected when the missing items were added to the first aid kit.

A caregiver who did not have a cleared background check was present at the operation and had access to children in care.

The operation does not have a current CPSC on file as the last form was signed on 1/27/2019.

After checking four children's files, there was no parent notification documentation stating the operation does not carry insurance.

The Pre- K and school age classrooms did not have activity plans posted.

The operation does not have a current fire inspection as the last one was conducted on 4/15/2020.

One out of four children were missing current immunization records on file.

The operation has been without a director for over a month.

Based on information received during the investigation, it was found that the operation had rodent droppings in the kitchen.

During the inspection, I observed infants, toddlers and pre-k children in the classroom with one caregiver which resulted in the class being over ratio.
