Accessible extension cords were observed in the school age room/cafeteria area. A cord was accessible in the three year old room but a shelf was moved to make it inaccessible. Several of the fence boards on the playground were observed to be loose or missing.
Two of the doors used as fire exits were locked and not easily opened from the inside.
