Caregiver failed to recognize and report significant bruising on an infant?s leg.

One of four child records reviewed admission assessment was not completed within the required timeframe.

One of four child records reviewed preliminary service plan was not completed within the required time frame.

It was reported that foster parents were pulling/yanking children by the collar of their shirts in order to get the child to comply.

Service Plan did not indicate how a child in care would be supervised when walking home from school and in certain instances having to wait outside unsupervised for the foster parent to arrive.

A review of a child's records revealed that the foster parent was not following the child's service plan. The service plan outlined what the foster parent was responsible for with regards to the child's medical diagnoses.

One of four home records reviewed only had one unnannounced quarterly visit for the year of 2021.

The agency did not report a child's illness and medical treatment/hospitalization until 3 days following the caregiver informing the agency of the incident.

Caregivers did not report a child's serious illness, urgent care medical treatment, and hospitalization to the TX Abuse and Neglect Hotline. Caregivers only reported the illness and 2 day hospital stay to the agency a day after being released from the hospital.
